I scheduled a massage therapist to spend the day at our office to give the staff a treat for Administrative Professional's Day. The therapist came the night before to set up and finalize the schedule. Each employee was given 20 minutes to spend with the massage therapist - with most choosing to do a fifteen minute chair massage and a five minute foot rub - before returning to work. The therapist charged her hourly rate - $50 - for an 8 hour day, for a total of $400.

The chair massages and foot rubs were amazing. The massage therapist was very professional and adhered to the set schedule during the day, and offered to stay a few minutes late to finish up a couple of employees. Some people just don't like being touched, and will not take advantage of such an offer.
Tips
Find a respected and well-known massage therapist if you are looking to bring someone on-site to your office.
Do not let her put out a tip jar - she is getting paid by the company - and any gratuity that is expected should be negotiated down and paid for by the company.
$50-$60 per hour is a reasonable rate. I would not pay for more than that or any hidden fees such as a transportation or equipment fee.
